Please note that this facility is for use after specific direction from a member of the practice team only. Any uploads which have not been specifically requested prior to sending will be deleted before being opened.

There is a 10MB limit on the size of a file which can be uploaded. Please do not send photographs with any identifiable features (e.g. pictures of a whole face) or of intimate areas of the body.

The picture of the area is to be taken at the best definition your camera allows ensuring there is enough light on the area and that the picture is clear and in focus.

You should complete all parts of the form, including details of the clinician who has asked for the photograph to be sent to them.

Once we receive your email and photograph, we will acknowledge receipt.

Please be advised that the GP will contact you when he/she has viewed your photographs to discuss any treatment.

This is to confirm you are registered with the practice, to allow the practice team to contact you and also to update your medical records held by the practice and our partners in the NHS. The internet is not a secure place, however, we have gone to great steps in making sure the information you submit to us is as secure as possible. We use SSL (Secure Socket Layer) certificates to encrypt the communication between your computer and our web server.
