Certificates
About Medical Sickness
If you have been absent for greater than 7 days from your work (usually 5 working days) you will need to obtain certification from your doctor.
If you are absent from work for less than 7 days, you only require to complete an SC1 form (a self certification form) and forward it to your employer. These are available from our surgery.
Types of Certificate
Form Med 3
This is the usual certificate issued by your GP. It states the condition causing your absence from work and the length of time you may be exempt from work. The certificate must be issued and signed by a doctor and therefore an appointment is necessary to obtain one.
Form Med 4
This is a more detailed statement about your illness and is again only available from your GP.
Form Med 5
This is a retrospective version of form Med 3 if there has been a period of time not covered in the past by certification.
When are we unable to issue a certificate?
When there is no documented evidence of illness in the patients case notes
When you have not been seen by a doctor in the Practice.
